Work Permit Information
Minor Work Permits can be obtained at Boone-Winnebago ROE 4 THE SAME DAY during regular business hours Monday through Friday, 8:00 am to 4:30 pm.
Students and their parent/guardian should:
1. Visit www.roe4.org
2. Hover over SERVE along the top of the Home Page
3. Click Minor Work Permits
4. Carefully read ALL instructions
5. Read (and download where applicable) the SIX requirements at the links at bottom of the page
6. Student AND parent/guardian WITH IDENTIFICATION should bring ALL FOUR completed documents/required forms to the Regional Office, 300 Heart Blvd, Loves Park, IL 61111, to complete the Minor Work Permit process
