South Beloit High School Motor Vehicle Registration Form

You must be signed into Google with your student South Beloit email address to successfully complete this application. You will know that your application has been fully submitted when you receive a copy of the completed form to the student's South Beloit email account.

Please ensure that you have the following documents scanned and ready before beginning this application as you will be required to upload these documents before submitting your application:

-Valid driver's license for student.
(You cannot apply if you do not currently have a license -- permit application will not be held until license is obtained.)
-Valid and up-to-date insurance coverage (for vehicle to be parked on campus).
-Completed 25-26 Consent Form signed by student driver(s) and parent/guardian (YOU WILL GET A BLANK CONSENT FORM FROM THE OFFICE).

Proof will need to be legibly uploaded for all documentation requested and answer all questions required below; OTHERWISE, your application will not be accepted - No exceptions.
